Based on a new failure, FM Approvals has widened the scope of this concern to include all SPP Pumps Ltd. (SPP UK) produced diesel driven pump sets utilizing the Centaflex Model A coupling sizes 140 and 200. The March 26, 2018 Product alert has been updated and the most current information is outlined below.
FM Approvals has been made aware of failures and cracking of diesel engine fire pump flywheel adapter plates manufactured by Centa and utilized by SPP UK.
These specific coupling components are susceptible to sudden failure which may result in the ejecting of coupling parts at high velocity. This sudden failure will disconnect the drive capability of the fire pump and creates a personnel safety hazard. A review of the manufacturing history has indicated that the majority of the affected pumps were sold within the European Union but there are units known to be in the Middle East and Asia.
The coupling assembly consists of a Centaflex A coupling, a spool piece and the flywheel adapter plate which is mounted to the engine. (See Figure 1). There is evidence that coupling misalignment is a major contributor to the rate of adapter plate failure.
FM Approvals recommends the coupling be replaced for all SPP UK manufactured pumps which utilize the Centaflex Model A coupling sizes 140 and 200. SPP UK has coupling replacement kits which have different style couplings, thicker adaptor plates and a greater degree of acceptable misalignment which can be retrofitted to existing installations. When any fire pump coupling is replaced the pump and driver must be properly aligned to within the coupling manufacturer’s tolerance to ensure reliable operation.

Hazard Involved: The adapter plate, when subjected to coupling misalignment, may result in a catastrophic failure (See Figure 3). The failure results in the loss of power transmission between the diesel engine and the fire pump. This failure will result in the loss of the water supply to the automatic sprinkler system. Additionally – This failure mechanism creates a personnel safety hazard. Portions of the engine adapter plate and coupling may become dislodged and eject at high velocity.
